Overview

Are you passionate about helping patients and families during life's most important moments? As a Hospice Weekend Intake Coordinator, you'll play a critical role in connecting patients to compassionate hospice care by serving as the first point of contact for referrals and coordinating the admission process. You'll work closely with referral sources, patients, families, physicians, clinical teams, and branch leadership to ensure every referral is handled with urgency, accuracy, and exceptional customer service.

**This is a Remote Position - Saturday & Sunday 8 am - 8 pm EST**

Responsibilities

  • Coordinate referrals from initial receipt through handoff to the Admissions team, ensuring timely follow-up and exceptional customer service.
  • Schedule hospice evaluations and admissions based on patient needs, staffing availability, and geographic coverage.
  • Communicate professionally and compassionately with patients, families, referral sources, physicians, and internal teams regarding referral status and next steps.
  • Support insurance verification, authorization requirements, and referral documentation while identifying and escalating barriers to admission.
  • Maintain accurate documentation in the electronic medical record and monitor assigned work queues to ensure timely referral processing.
  • Collaborate with interdisciplinary teams to improve referral conversion, reduce delays, and enhance the patient and referral source experience.
  • Maintain compliance with HIPAA, Medicare regulations, company policies, and applicable hospice standards.

Qualifications

  • High school diploma or equivalent required; Associate's degree in a healthcare-related field preferred.
  • One (1) to three (3) years of experience in healthcare intake, admissions, referral coordination, customer service, or a related healthcare environment.
  • Hospice, home health, post-acute care, or hospital discharge planning experience preferred.
  • Experience with insurance verification, payer authorizations, Medicare, Medicaid, or commercial insurance preferred.
  • Strong communication and customer service skills with the ability to interact compassionately with patients, families, and healthcare professionals.
  • Excellent organizational skills with the ability to manage multiple priorities while maintaining accuracy and attention to detail.
  • Proficiency with electronic medical records, Microsoft Office applications, and other healthcare technology systems.
  • Knowledge of HIPAA, patient confidentiality requirements, and healthcare documentation standards.
  • Ability to work independently while collaborating effectively in a fast-paced team environment.

$21.00 - $25.00 per hour (Average Pay Range) plus bonuses! The pay range listed represents a general guideline for the role and is not a guarantee of the final offer. Compensation will be determined based on the selected candidate’s relevant experience and the specific responsibilities of the position. Final compensation rate will be discussed and confirmed at the conclusion of the interview process.
